Tissue-type-plasminogen-activator-associated fibrinolysis in orthotopic liver transplantation.

چکیده
ORTHOTOPIC liver transplantation is frequently associated with hemostatic disorders and a bleeding diathesis.I.2 Accelerated fibrinolysis has been recognized as an important component of the hemostatic defectY The origin of this fibrinolytic activity, however, is still unclear. We investigated the role of tissue-type plasminogen activator (tPA). a potent activator of the fibrinolytic system, in the origin of increased fibrinolytic activity during orthotopic liver transplantation.

Systemic effects of tissue plasminogen activator-associated fibrinolysis and its relation to thrombin generation in orthotopic liver transplantation.
Orthotopic liver transplantation is frequently associated with hyperfibrinolysis, the origin and clinical relevance of which is largely unknown. In 20 orthotopic liver transplantations, we studied the occurrence and systemic effects of hyperfibrinolysis. Severe fibrinolysis was defined to be present when the euglobulin-clot lysis time and the whole-blood-clot lysis time, as measured by thrombel...
متن کاملFibrinolysis during liver transplantation in humans: role of tissue-type plasminogen activator.
Human liver transplantation is frequently associated with a coagulopathy and bleeding diathesis developing during the anhepatic phase of surgery. The hemostatic defect has been attributed in part to accelerated fibrinolysis. In this study we evaluated changes in specific blood fibrinolytic parameters occurring in eight adult patients undergoing first-time orthotopic liver transplantation. Five ...

BACKGROUND The long-term risk of thrombotic and vascular complications is elevated in liver transplant recipients compared to the general population. Patients with cirrhosis are in a hypercoagulable status during and directly after orthotopic liver transplantation, but it is unclear whether this hypercoagulability persists over time. AIM We aimed to investigate the hemostatic status of liver ...
متن کاملBlood conservation in liver transplantation: The role of aprotinin.
ORTHOTOPIC LIVER transplantation has become an accepted treatment for end-stage chronic liver disease, with patient survival rates of 80% to 90%.1 In addition, it can be a life-saving procedure in patients with fulminant hepatic failure and an effective treatment for several metabolic disorders. Currently, a total of about 10,000 liver transplantations are performed each year in the United Stat...
متن کاملThe pathogenesis of accelerated fibrinolysis in liver cirrhosis: a critical role for tissue plasminogen activator inhibitor.
The pathogenesis of accelerated fibrinolysis in liver cirrhosis was investigated by comparing the results of specific assays for tissue plasminogen activator (tpa) antigen, tpa activity, tpa inhibitor, and alpha-2 plasmin inhibitor (a2PI) in 12 patients with cirrhosis and markedly accelerated fibrinolysis (dilute whole blood clot lysis time (DWBCLT) less than two hours), in nine patients with c...
